+T0.3 决定：因 T0.1 的 0001_init.sql（SELECT 1）可能已写入已有库 schema_migrations，完整表结构放在新增的 0002_schema.sql，不改写 0001 语义。schema_migrations 表由迁移执行器 CREATE IF NOT EXISTS 维护。写入队列 T0.3 用互斥一操作一事务，合并提交留给 P2。
